• A completed and signed application form and 2 essays.

• A copy of your high school transcript.*

• A copy of your school’s profile.*

• One letter of recommendation from a teacher or guidance counselor.* 

• IF YOU ARE APPLYING FOR THE SWIMMING OR FIELD HOCKEY SCHOLARSHIP also include- One letter of recommendation from a coach.

• *ask your guidance department and or teacher to provide items #2, #3 and #4 to you in a sealed envelope, or mail them directly to the address below. Give your guidance department at least 1-2 weeks to process your request!
